I need a simple form that will look good on a small screen iPhone and also a big screen iPad. The form will collect NAME EMAIL CITY PHONE NUMBER phone number will be a formatted 10 digit max, and email will need validation for properly formatted. This will take the data and post it to our existing base and will then revert form back to a page that says success, and then auto redirect to the form page. The form page will look neat and clean and also contain a couple lines of text and our logo. You will use the existing config file to connect to our base, instead of creating a new one.
