I need a website built that contains information about different types of insurance, though the focus is to ultimately generate zip code submits on a lead gen form (see [login to view URL] for a rough idea, though I like the design of [login to view URL] and [login to view URL]). The site should be built on Wordpress. It will have a homepage similar to what you see on [login to view URL] - large top graphic with a zip code submit box, then content below it. I will NOT need a full lead form built (the zip submit box will go to an affiliate page). It will have several category pages for different types of insurance (auto, health, life, etc) Under each of those category pages there will be 50 or so individual content pages (I will provide the content). So basically I need: Wordpress theme implemented Basic design Logo Design Design of the lead form header (like you see on the top of the page at [login to view URL]) Content pages (the lead form header will be at the top of each page). I'll be providing wireframes to help with this...
