Website fix and email capture function

Fix the CSS in a website (simple formatting only) run on textpattern.. Then I want a pop up or slide in 'banner' with an offer to a free report or ebook and a form linking to a Mailchimp list and collecting first name and email and verifying these. This functionality needs to be able to be customized to appear on different pages and linking to different reports and form database lists. I want to own this code once done, no proprietary software.
