Need Database Scraped from Website to .sql file

I need the data from a Website scraped. The Website uses a city/state or zip form to search for doctors. The search results are a list of doctors, sometimes spread across a number of pages. The doctor names can be clicked to go into their profile and see more information about them. I need someone to extract all of the doctors for all cities/states/zips and all of the information about them into a structured MySQL database. I also will want the source code for the program you write to perform this task so we can re-check the site at a later time for new data. I will only disclose the Website in private messages, and I want exclusivity on the resulting database and source code. You will not be able to share the database with anyone else.
