Online Survey Script

We have an immediate need for a developer to create a survey script in PHP and Java (for survey creation Wizard) with SQL database). The script will be offered through a membership site (membership area is already set up. Members will be supplied HTML produced by the script. The survey results and respondent information will be hosted on our site 1)Members will be signed up at four levels, Basic, Advanced, Gold or Platinum the various levels will determine the number of responses they can collect. Members need to be granted access based on their purchase. 2)Members will sign in to their own admin area using the same password and user name that gets them into the main member's area. . 3)Members will be able to access their account to create/edit surveys, test surveys, publish surveys (produce the required HTML), view results in text as well as graphical form (bar graph and pie chart), view responses and respondent information and export all data to useable import format compatible with Microsoft Office products. 4)Survey look must be customizable for 10 different color schemes, custom color combinations and ability to add Logo if so desired. 5)The following is a list of the various screens and their functions that would be needed: [login to view URL] List Screen a. We will supply further question type details c. Set comments box on/off (a text field) d. IP/Cookie to ensure one-vote per user e. Date/time stamp f.
