HI, I have a WordPress site ([login to view URL]) built on a theme from woothemes with woocommece shopping cart plugin. I need a customized customer survey module. Requirements: a. Ability to conduct customer surveys. [login to view URL] contain multiple questions of type –multiple choice, rate from 1 to 10, open ended – key in answer. c. In each survey I would like to show her a configurable number of my products and ask multiple questions about each product shown – ideally all questions related to 1 product be on 1 page. d. In each survey I would like to show her a configurable number of items which are not my products yet (which I am planning to order in a future consignment and ask multiple questions about each item shown – ideally all questions related to 1 item be on 1 page. These should not get listed on the gallery page even though they occur in the survey. e. I would like to run simple reports on the above data to determine which are my most popular products / items and which are least popular. f. I would like to segment the data by users'age, income level, race and other parameters – race is determined by surname (Indian, Chinese, Malay, Caucasian, others) g. Each survey can be conducted in person or on the web on their own. We must be able to record if the survey was conducted in person or on their own. h. Date and time of survey must be captured. [login to view URL] can be taken only by people who we ask. Typically we will give them some kind of brochure which has a link to our survey page to take the survey. Other people cannot access the survey page - protected. j. Survey's have an expiry (configurable). k. After taking the survey each user becomes eligible for a voucher or gift certificate. Add voucher to user's account and allow him to print the voucher after we mark the survey as reviewed. l. I will also have customer satisfaction surveys (after shipping it completed). An email should be delivered with the link to this surveys in the users mailbox a configured number of days after the dispatch of items (i.e. when order status indicates dispatched from our end).
