Sql Query with MySql

Brief summary, details in attachment. I will need help with a mysql query to paste into excel 2011 using the mysql obdc driver. I have that working properly to pull the tables, but I don’t want to pull all that data. Then I need help correlating the names to ids and performing counts etc as discussed in the attached document. I will consider using php script if I am able to export the data. I have attached a work document listing the tables, the columns, and the relationships.
