Database and Java coding for dental office records

This project requires you to computerize a dental office's partient record sheet, which is a 10-page paper document. I want it to be converted for entry into a MySQL database in a client-server environment. The details of the project and scans of the 10-page patient record are attached. This is the first of several similar projects I need doing, all in mySQL and AJAX technologies for client-server environments. Consider this a test piece: if you can do this correctly, you will receive the contract for the other pieces and, potentially, a full or part time position maintaining the code for at least two years (if you want to do that) with a retainer paid every month. I have had some bad coding experiences on RAC so want to find someone who I can rely on to do this first task properly, then go on to the more complex tasks in the larger system. Deliverables for this project are a complete, working application that is described in the attached document. Note that the ZIP is large because it includes scans of all the patient record pages. If you need higher-resolution images I will provide them on an FTP server for access. In your bid do not just say "I can do it" and that's all. Tell me which languages and technologies you will use to complete the task. Also, I need to know a realistic timeline for delivery of the final product.
