Customer Feedback application

Customer Feedback application this application very simply will collect customer feedback on the product and services provided by the company. the design should be flat design with layers over each other exactly as IOS7 layering plus the ability of navigating between the question layers exactly like safari navigation on iPhone between the pages. first the user should save his/her credentials as name, phone number, age, and address.(no user name and password required) then the client will choose a location from the map, where the branches will be flagged in addition to a drop-down menu of all branches. it will have four layers of questions to be answered with a nice animation each layer will contain 8 questions with 4 choices 1-Needs improvement, 2-Fair, 3-Good, 4- Excellent against each question. once the application is installed and on every time running it should have an intro like Skype bubbling animation as you run the app. once the feedback is submitted it should be saved on a database we will provide. there should be save questionnaire for completing later and send when internet connection is available. the priority will be for those freelancers who did something similar and please send us any project near or similar did before. we will need to see the designs before approving any freelancer for the job.
