Custom Dynamic Lead Submission Form With HTTP Post to External DB and Save to Internal DB.

#READ BEFORE YOU BID# We need an Online Lead Submission Form for Call Centers to use and submit Auto Insurance Leads. The form will be dynamic and will have several functions a normal web form does not usually have. Form needs to have several sections with required and non-required fields. Non-Required Fields in each section have to be hidden with a button "DISPLAY OPTIONAL FIELDS" in each section end to slide open those hidden fields. Form will also contain many field validations such as age, dob, zip code match city/state, year/make/model/trim of the vehicle/car, email validation with deliverability confirmation, phone number validation and more. Form will also contain several conditional fields. ie: If 1=a and 2=b display question#4 as required. etc... The form needs to be connected with a DB that we will provide to pre-populate Years/Makes/Models/Trims of the vehicle/cars. Another DB will be to pre-populate city and state against the zip code entered. The form will have an internal Database with admin to pull reports. The form will need to post the data to an external site/database and capture response for each processed lead using HTTP Post/Get Method. API Posting Instructions will be provided in details. The form will also need to be connected with DropBox API to upload recording files for each lead being submitted. The person submitting lead will have to Browse/Select/Upload a recording file which needs to be uploaded in .MP3 format to the DropBox and capture/save the link for each recording in internal DB and post that link to the external DB as well with rest of the lead data. we are not too familiar with this process but know that this is possible and not too tricky at all. DrobBox provides full API to do this. Once a lead is posted using HTTP String to the external [login to view URL] external site will send back an XML response which will look similar to the one below. IMPORTANT: The form has to be dynamic meaning it needs to capture some fields from the Landing URL. Each call center/source have different User IDs and Campaign ID's and without those we well not know who submitted the lead. there will be 2 identifier hidden fields which will be identifier fields. Example Fields: AID, VID. Form main link= [login to view URL] We need the form to capture these fields from URL. so the landing link can be something like: [login to view URL] So the form needs to capture AID and VID from the Landing URL Also there will be several other hidden fields. Some will need to have static values pre-set in the system but some will have to be captured dynamically such fields will be like Date/Time Stamp in US EST timezone in MM/dd/yyyy+hh:mm:ss format, also need to capture IP Address of the user for security purposes. System will also have the following duplicate checks: - Email - Phone - Phone+Vehicle Make/Model/Year/Trim - Address+Vehicle Make/Model/Year/Trim Duplicate Check will be for past 30 days only. #Designing Also Required#
