360 Assessment Report Generation

We currently have two related assessments on two separate Google Forms. One is for a Self-assessment of an individual and the other is for a Rater-assessment for that individual. The output of the Google Form is in a Google Sheet (each form has a Google Sheet attached to it). We would like to take the resulting data from the assessments and put the data into a PDF report based on a structure we will provide. It will be one report and have aspects of some of the reports attached but basically have the same structure of these reports below. Some of the graphics below are samples of different graphs we would like in the report. We would like a mechanism that allows us to generate a report based on the self and rater results. We are flexible on how the report is generated and data is send to this report however we are limited to whatever we can produce from Google Forms/Sheets so we could copy this to a MS Excel spreadsheet, CSV, text file..etc. We will eventually have this all on a website from the taking of the assessment to the generation of the report however we have an immediate need of a report at this time. So whatever we need to do manually to generate this report we are prepared to do for the sake of our timeline but we need to begin to generate reports of each of these assessments within the next few weeks. In the next project we will look to automate this and put this assessment on a website.
