I need a Scholarship form created that checks the answers to the matrix that is attached and at the end of the form tells the user what scholarships they are eligible for and then sends that data to a recipient. As well as stores that application in a database. This needs done right away. All the questions are in the attached file and each scholarship has a y next to the question that they must answer yes to be eligible for that scholarship but some scholarships require multiple yes answers to qualify for them.
