HTML5/CSS3 Website and Rebate Calculator Form

Freelancers, I am looking to develop a simple secure login website that will allow users to fill out simple input forms to calculate and summarize rebate values and email .pdf reports of calculation results. Users will login and fill out a 1 page form. The form will use the input information to perform a simple calculation to estimate rebates for performing energy upgrades. The user can then email the form results as a .pdf to their email address. The recommended development language for the website and forms is HTML5 & CSS3. The website and form should be modern, clean design, fully responsive and process/display summary calculation results in real-time.
