Tablet application (Andorid and iOS) for financial consultant

Objective: A Financial consultant goes to his client with a long, boring form called 'client fact find'. This is a lengthy process to write. We want to convert this form into an app, which the agent can take with him and input the data. All the information entered is synced with the server. There is no CMS needed. Scope: (1) All form fields to be converted into pages as a tablet app (2) All data to be saved to a db on the server (3) Each form will have a unique ID and can be retieved by fetching data from the server (4) Each page on the app will have a unique background image (see prototypes), which can be changed easily (not though a CMS, but maybe by just replacing the file) (5) On one of the pages, there will be an option to use the tablet camera and take a picture. This picture will be just placed beside the form fields of that page only. This feature is needed only for one page, but is necessary. (6) Some of the data will be taken an inserted into a formula at the last page and shown as a graph or simple calculations. Please see this for example: [login to view URL] What will be provided to you: (1) Design Direction and protoypes (2) UI kits (3) We will provide apple dev account and google play account What is expected from you (Deliverables): > Create the app and publish it on Google play and iTunes store. > Only for tablet, not for smartphones To make it easy for you: This is not a public app, it will be used only by the consultants. So you can create this even as a responsive website and then just package it as an app using Phonegap or any other framework.
