Update php MySql software

I have software that was custom designed some years ago using php (5.0 I think) and MySQL. The software needs updating to recent php and MySQL standards. The software handles referrals for counseling services. Referrals are sent from 1 customer/agency. The system is mainly used by provider of services. Referring agency mainly has view/only and access to data that is input by the provider (view date of visits for a specific service for a client, review activity reports for any visit(s), view budget remaining info, view hours remaining, etc.) System does the following: - Allows input of referrals (using an online form that needs to be updated to be a replica of the pdf referral received from customer/agency) - System must allow referral for single or multiple services/programs (radio buttons to select services, etc) - There is a set list of services available. Each service/program hourly cost is to be entered by Admin; Program/Service maximum budget can also be entered by Admin - Provider Admin assigns cases to provider worker. The provider worker clicks on the client’s name and enters a report through the system each time they give the client a specific service - activity report form allows worker to input dumb data about interaction with client. - Activity Reports also collects smart data such as: (start time, end time, date, begin address/mileage, ending address/mileage, etc.). This allows the system to bill the customer based upon the hourly rate of the service, system calculates the provider workers pay, mileage report is completed b/c we travel to the client, and shows how many hours and money are remaining in the case). Basically, the system needs to be updated to allow for (system already does these things, but we want to make sure it is working properly): - Addition or deletion of service/program (when a service/program is added, we can “Name” the service and input the hourly rate for the service, the maximum budget for the service, etc) - Each service/program should be listed under the provider worker’s profile so that we can assign their hourly pay for each time they provide a specific service (remember, there may be different pay scales for each service for the worker).
