Hi! i need a survey app very easy that guest the PDF question here attached when user get into the webpage ask: - Name - Age - Nationality - Course (listbox field) after goes on - make the questions (i need possibility to add/remove questions) when finished - make a calculation - store the datas in a mysql database - shows a "graphic" The result should be similar to this [login to view URL] Of course I should be able to: - set the questions - set the value of each question Please in the PDF u will find the Explanation how it works (first 2 pages) Example of this is in this link
