I need to add a small module existing website. I need a simple form to be developed using which a user can share a YouTube link. For Example a teacher is required to share a video he will mention a class for which the video is meant the subject, lesson learned, description and verify his email and his video will be displayed in search results when search is done on basis of subject lesson or teacher. So the fields are Name, Subject, Lesson, Description, video link and on submitting email is verified and video link is posted. search results be shown on basis of teacher lesson and subject
