pdf generation in php

I need following attached pdfs to be created and saved to a folder in php. Must be editable. I'll will replace values for textboxes, checkboxes with variables in php later. Use default values for these fields for now. You can use any library like tcpdf,mpdf etc. I don't care about the method you use like html to pdf or create these fields in pdf. I need the exact replica of these pdfs So I can replace it with actual values. No up fronts please. I'll fund & release after the job is completed. I'll give you very good feedback if you do well.
