Modification to a form on Magento 1.9 community

The project is to improve an account request form on our Magento 1.9 website. The form looks great, but is asking too many questions, making it too long to fill up. We want to remove a lot of fields, and hopefully get the form to go from 4 pages or more, to only one page. -The fields' answers are added to the 'account information" section of the admin panel. The fields to be removed on the form will need to be removed from the account information section as well. We would like to keep our DB clean. Some fields (Country, State, Province) should also be a drop down menu and need to be corrected. Some places have incorrect color scheme: white writing on white background, or dark writing on dark background. You must be capable of adjusting these details without requiring instructions from us, just like if you see that a field is not aligned with the rest of the fields, it should be adjusted as well. It is the same with the English mistakes you may find throughout the process. Example: When selecting "create an account" on our website, a pop-up appears, that says: "Which language you want to select?" (this line is white, and the background is white: it is difficult to see it) - this one line should be made visible (not white/white) and should say "Please select a language". ** Optional: we would like the form to maintain the answers in the fields, if the person goes "back" on their browser, or leave the site to come back later ** The purpose of the online form is to replace a PDF file we have customers fill up when they want to start buying from us. Once the form and admin panel are updated, the form will have to tested successfully. This is a very small project, for someone who is comfortable with this type of work.
