Looking for web designer to create custom purchase form. I'm looking for a web designer to create an order form that looks basically like this: [login to view URL] Not so concerned with how it looks--I can tweak the CSS--but need something that works. Form will dynamically calculate and display price based on the user inputs. (Currency won't need to be calculated, only word count). Will need form to continue to next page to gather information, and then go to a check out page. Will be implmenting on my WordPress site.
