Need to convert form into online form

Queries/Confirmations: 1. Will the same form to be added to multiple sites? 2. Will the form added in multiple sites to be sent to a single email address on submit? 3. Will all these websites be running on different Servers? 4. We can then build the form as JSONP + Ajax or iFrame+Ajax solution (whatever we find suitable at the time of development and answers to all these questions). 5. The form will be processed by a php program through Ajax that would be running on your Server (on a single Server) 6. After submission of the form, a PDF will be generated containing the submitted data received by the above server script and will be emailed. 7. If someone likes to print blank form (we will either allow printing the same html) 8. We need access to your server where the main php script would be hosted and access to one of the website servers where the form frontend would be added. 9. Also need SMTP details of the email account from which all the emails would be sent from your server. 10. what version of php runs on your main server ? 11. Can we support the latest HTML5+CSS3 supported browsers or any specific browser/version you would like to support ? Above details will enable me to assess scope of work precisely. I think it will be more convenient to you if script generates a PDF file with details filled by user in form and this PDF will be attached in email along with details in email as well It will provide a clean layout arrangement with clear data mapping
