I need to get 3 forms created that I can insert into 3 pages of my Wordpress Theme (as per attachment [login to view URL]). The theme is called SociallyViral by MyThemeShop. The idea of the forms is that users can submit suggestions of content. Two of the forms are for submitting suggestions of Quizzes and the third for picture lists (images and captions). As per the attached pdf ([login to view URL]), which has the basic framework for the 3 forms, the 2 main elements of the forms are text boxes and buttons to upload images and gifs. I will be uploading the data from the forms to my site manually so I simply want the data from the 3 forms to be stored in separate databases. I would like the forms to have a flat design. I have attached screen shots of the sort of flat form design I would like my forms to look like (flat form [login to view URL]). The framework designs I have done in the pdf are in English, however the actual forms will be in Arabic, so the forms will need to support Arabic and RTL. I will be providing the Arabic translation.
