I have a form on paper that I would like made into an online form. Using Gravity Forms, you will create an online form on my WordPress site. Creating an exact replica of my form. The user must fill out all portions of the form save 1 or 2 lines. The user must choose, "Sign Digital" - in place of signature at the very end. I believe there is a Gravity Forms to PDF plugin that can be used. This is a one page form (8.5 x 11) Once the user fills out the form, a PDF copy is sent to their email and to my email. YOU MUST KNOW HOW TO USE GRAVITY FORMS!!! THE FORM MUST BE RESPONSIVE LIKE MY SITE!!! MY LOGO NEEDS TO BE AT THE VERY TOP OF THE PDF FORM!! YOU WILL DRESS THE PDF FORM TO MAKE IT LOOK PERFECT. THIS PROJECT WILL NOT CLOSE UNTIL THE FORM IS ABSOLUTELY PERFECT.
