Add forms to Wordpress site - Relist

We're updating one of my websites here: [login to view URL] I've got a couple of forms I need adding and connecting up to my Aweber account. At the top there under the Hero image, you'll see a blue banner. I need one form in there that should look like this: [login to view URL] I've got as far as setting up the main container Div in the [login to view URL] file (hence why the background is there already, although you can start again if that's better for you, I don't mind), but I just don't have time right now to set up the column divs for the image and the elements. I've already uploaded the images though (including the "Free download" text as a PNG because I know what a pain in the arse that would be to do as css!) and I also have all the styling details (colours, borders, radius's, etc.), the form just needs constructing. Then as you scroll down you'll see two or three identical "Web Special" boxes. These just need a functioning "Email" text box and submit button under the text. You can see an example of these in action already at [login to view URL] (in fact you can rip the validation code right out of there if you like!) with the "Free workout" boxes. Let me know if there's anything you don't understand and I'll add answers to questions to this project description. :-) I've had to re-list the project because it turns out the winning programmer last time couldn't speak English, and because it's impossible to make the instructions any simpler it was going to cause untold amounts of time and stress fighting a losing battle and pursuing the project together. I'm not a programmer and I could do this project in a couple of hours, but I'm outsourcing not because I can't do it myself but because my time is better spent elsewhere. If I'm spending the same amount of time (or more in this case) having to micro-manage a project so simple, it defeats the object.
