Template Edits and Contact Form

I have attached a template that I need edited in Adobe Photoshop 7+. The site is a call center operations service for customer service, technical support, and trouble ticket reporting. I will include the .psd files, fonts, html images, and whatever else you need to make the edits. The edits will be as follows: 1. Integrate the included company logo into the site, can edit logo 2. Add text to the navigation buttons for each page with click effect 3. Add text elsewhere on page as headers 4. Centering template on web page instead of left align 5. Creating one page with basic contact form There may be a few more minor edits here and there. The graphic edits are all very simple edits that I am outsourcing because I want them done right. The contact form is a basic name, email, message with a scroll down of who to contact. This entire project should be completed this weekend. Please include "PSRINC" in your bid so that I know you read the entire project. Bids without this will be ignored. Thank you.
