Include a php page using JS

Hi, im looking for a quickly solution to include in any page a php page (page that contain a form with fileds that use jquery). I need to give to my user just a piece of code maybe in JS that include on their page a box with a form inside. Of course i cant use iframe cause the limitation. Scenario [login to view URL] (with all controls and form inside using js) user A need to include [login to view URL] in his page I cant use php include command i cant use iframe Need a solution for this
