Need PDF form created with expanding text fields

I need a PDF form created which can print onto medical note paper. The medical notes document is scanned and attached. The form must: - Be PDF - have a real-size background image of the notes page displayed on screen (but which DOES NOT PRINT) - have text aligned with the lines on the page, so that when it's printed it aligns well - when the text area on the first page is full, continuing to type should flow automatically onto the next page which is aligned with the back side of the notes paper (no clicking or selecting the next text box required), no matter how many pages are required - have an automatically generated time and date stamp in the appropriate column on the left of the first page - generate a "PTO" label at the bottom of each page that is not the final page, and generate a "(from over)" label at the top of each page after the first.
