Mini Application needed to run on IPAD

My client needs an iPad software built that will work when not connected to the Internet, so it cannot be a web based application, and it has to run on iPads. All we need is a simple application that is like a traditional Contact form on a website, and it just needs to have the client's logo at the top for branding, and then have these fields: Name Address City Zip Email We need the data to be saved to a csv file, or excel. (preferably csv) Then I require developer to write a document detailing exactly how to install the application on an iPad, since I am very unfamiliar with iPads and will need step by step instructions how to get it installed. Need the application completed by 4/12, so one week. Please indicate in your response that you understand the task and can complete it within 1 week, preferably sooner.
