Planner book formatting for Kindle Direct Publishing Print on Demand service

I have a planner that I need formatted for Amazon Kindle Direct Publishing Print on Demand. I encourage you to be creative in your design. I created a pencil concept image with the elements needed on each page type. Please note that KDP on Demand is the extra option past formatting for Kindle ebooks. It is the service that requires margin parameters for printing a physical book.

What I need:
1. Pages of the planner laid out. After the year page comes month pages. After each month page comes five week pages. After each week page comes seven day pages. (Note months have 4 weeks and a partial week to give the need for 5 week pages.)
2. Page size is flexible but should be 8.5 X 11 inches or less. Landscape format or square. I do not think vertical formatting would work for this planner.
3. Name of planner text in the top left corner stylized.
4. Place logo, I can provide the company logo.
5. Cover image as per Kindle Direct Publishing on Demand requirements (link above).
6. PDF files along with editable files in your choice of program. If using Illustrator or InDesign, save it in a format that the old version, CS6, can open, please.
