This will display one popup with 2 options "Create a business profile" or "Create Personnel Profile"
You are then taken to the relevant signup form.

I will outline this for the 2 registration forms - if its a business they will have the following fields to fill out:

CONTACT INFO
Country

Email
Website
Facebook Profile
AIM
BUSINESS DETAILS
Business Name
Address ( I would like this to be a Google Maps plugin )
Business Description
ABOUT YOURSELF

If It is going to be a Personnel Profile it needs to contain the following:

That will do for the Registration. visitors to the site will then need the option to view either "Tattoo Artists" or "Personal Tattoos"
The visitor to the site needs to be able to search the profile either Business or Personnel by filtering down by "Country" "State" "City" etc
I have already received a quote for this work so do not bid really high because I know how much this work will cost and the plugin allows you to set the fields that I am after.
This needs to be completed ASAP
