Design a Website Mockup

require to design a user interface design for P-EHR (personal electronic health record) which actually a personal health record system for obesity patients where patients can track their calorie level intake and intimate user through a message to mobile phones when they cross their intake level.(no need for running the application). There are some other requirements which can be discussed later.
