PDF to Word OCR and Manual Review & Correction

We need a 10 page Legal document scanned into a PDF turned into a word document. We don't care how you do it but the finished version should be in an equally acceptable format & Layout with similar fonts. It does not need to match 100% but needs to look professional. In other words, if someone wants to re-type from scratch that's fine as long as it looks clean and professional. It should be checked word for word for errors prior to submission to us as completed work. Every 3 errors will equal 1 point off of feedback. text items that are added to the document such as the signatures, initials and stamp are to be removed completely. Basically we are looking for a blank form (from one that was used) to be re-used over and over.
