Develop and implement a new design for our login area

I need someone to make a new design for my login area according to my specifications (see attached file) and implement it afterwards. You have to be familiar with using git and stick to the git workflow. So you do not work on our server, but clone the dev system and just commit tested changes to the dev system. It is absolutely necessary that you know how to work with git and we discuss changes on a demo version in development on your server. You only commit working versions on our server that are final or almost final. It is important that you at first make the design layouts and then implement them with the described functionality. Furthermore I need to know a deadline when my project will be done. I can work with 3 days maximum until work is done.
