Connect form to mysql db

There are few parts for this project: 1-I need to connect a form created by [login to view URL] to my mysql database and post entries into my mysql instead of using formassemblys posting action. I have the database but it may need to be changed.. There are minor changes needed for the form also. You can login and test current form at: [login to view URL] User: test(at)[login to view URL] PW: 1234567 After submitting the form the user taken to a page where he can continue out of the website to make payment by paypal. 2-Under the application status page the user should be able to see the status of his application. If the status is not "closed" he should be able to edit his information. User should be able to continue filling a former application that he did not finish. 3-I will also need a page under the admin section where I can pull the applications in a similar format as the form.
