Change form from Contact 7 to Gravity Forms

[login to view URL] Task #1: Here you can see a form over the top of the slider. This is generated using contact form 7. My client wants to use Gravity Forms instead. I need you to recreate the gravity form to look exactly like the contact form 7 that is there now. Task #2: You'll notice on the About Us, Testimonials, and Our Company Terms pages, they all have a different page template than the other pages. I'd like ALL pages on the site to be the same. I'd like the Contact Form to be in the same place on these pages as well, instead of the bottom right column where it is currently. Please keep all the pixels on the pages they are currently on, if for any reason they have to be moved, let me know so I can make sure everything is setup correctly when we are finished.
