asp to pdf

I need ASP code (or any code) that will pass html form inputs to pdf document form fields) I am wanting to use an html form to fill (find and replace) form fields in a PDF document. The pdf documents are already created and are templates stored on the website. After logging into the website, a landing page will offer the user several PDF document options. After selecting the PDF document the user is directed to a another webpage with an html form that askes several questions (text inputs, drop downs, radio buttons)pertaining to the requested PDF document. After completing and submitting the html form the application will create the PDF document with the user inputs and ask if the user wants to save or open the the pdf. The pdf documents are always changing and the source code will need to be changed and modified as new pdf documents are created and used. I understand ASP a little but will use any code that I can change and have full control over.
