Contact Form/Graphic

I am looking for a contact form developed in Flash. It is a cross between a contact form and a small animation. The form itself, speaking of the fields, should be easily editable wheras I can add/delete/alter all form fields. The UI of the form originally should appear to be a piece of white paper asking for specific information from the user. More easily put, surrounding the form which the user adds thier input will look like a piece of paper. At the bottom of this form will be a button/link entitled 'FOLD'. When the user clicks this button the form must be checked for required fields (to be discussed later) and if all fields are okay the paper will fold itself into the shape of the envelope. It will fold inward from the viewers point of view and it will flip so you can see the front of it. Here comes the more tricky part of this...You need to know how an actual envelope works as how it is to be prepared for sending: From on upper left and To in the center. Depending on what they add on the form, this will change. For an example, if they mail the form to me (which will always be the case so this part will be static) it will say 'EDT' 'email address here' in the center part of the envelope. And whatever their name and email address is, that will be presented on the upper left corner saying their name on top then their email address. At this point the form must be verified and ready to be sent. This envelope must be draggable ontop another image (designed by you) which will be a mailbox, not the kind at your house, but the ones at the post office. The user drags the envelope on top of this and releases and the enevelope dissapears into the mailbox. At this point I was thinking of having an animated mailman on a motorbike ride out to pick it up, but that may make this more expensive. If you have a creative idea on how the mail can be picked up without costing an arm and a leg, let me know! In any case, they will be alerted by some means, either by a popup or the mailman waving his finger, that there is something missing on the letter which it needs to be sent. What is this last thing? Yup, a stamp! The user will then be prompted if they want to add a stamp, and the user has to click YES. Now a selection of stamps will appear on the screen, 3 small images (which you will design) which when clicked will add itself to the envelope. (Once alerted they need a stamp, the folded letter appears back in center of screen).
