Create a form using Gravity forms on a wordpress members site. The form is called “Service Calculator” The form is intended to calculate the cost of a service by unit of measure (UOM) for a particular product where the cost is typically a lot charge or cost per pound, or a combination of both. For example, If I have a 9000 pcs (or 9M , UOM = “M”) of a part that weighs a total of 180lbs (for the 9M pcs) and the service company is charging me a minimum lot charge of $55.00 or $0.25 per pound (lbs) (whichever is greater), then the form should calculate which is greater and use that cost to calculate the cost per UOM. User should be able to add new lines for additional services using the same calculation. I have an excel spreadsheet I can email you that has the formulas and should be easier to understand than my explanation above. The form will need to recognize the current logged-in user The form will have the following components Dropdown box with a list of existing “Service Calculators” for this user Buttons Buttons next to the dropdown box for Load existing Service Calculator (selected from Dropdown box) Label a label with current date Fields 01-Service Provider Name ( text 30) 02-Service (text 50) 03-Cost per Lbs (Number) 04-Min Charge (Number) 05-Number of pcs for service (Number) 06-Total weight in Lbs (Number) 07-Total Cost (calculated) (=IF(Min Charge>(Coster per Lbs * Total weight in Lbs) then Min Charge else (Cost per Lbs * Total weight in Lbs) 08-Cost per unit of measure (calculated) (Total Cost / Number of pcs for service) Buttons a button to add additional Services. By clicking the button, the form will generate additional fields of Service, Cost per Lbs, Min Charge, This additional information will be included in the calculation for Total Cost and Cost per Unit of measure. a button labeled “calculate” which will calculate the above formula and displace “Total Cost” and “Cost per unit of measure” fields a print button to print the information above along with the date and user name Save buttons- The following two buttons will allow the user to save the preceding form with all the information entered for future use, except the “Number of pcs for service” and “Total weight in Lbs”. A “Save as existing calculator” button to update or save preceding calculator under the same name, this button should be faded if there are no existing calculators A “Save as new Calculator” button which will allow the user to save the preceding form and any modifications under a new name Please see attached Excel spread sheet for formula example
