Address book

Deliverables [login to view URL] project will consist of a completely searchable address book with a graphical user interface written in javascript. 2. It must include a name prefix which should be a drop down list and includes Mr., Mrs.,Ms., or Miss 3. the first and last name must not exceed one line or 32 characters 4. The middle initial must not exceed two characters. 5. The name suffix must be a drop down list. It should include Jr. ,Sr. or roman numerals. 6. The business name must not exceed one line or 32 characters. 7. The business address must not exceed on line or 50 characters. Font size assumed 12 point or smaller 8. All the phoe numbers should be two different feilds One feild is the area code, the other feild is the rest of the number without hypen. 9. The birthday must be in the following format: MM/DD/YYYY 10. The main email address must not exceed one line or 50 characters 11. The webpage must not exceed one line or 50 characters 12. The additional notes must not exceed five lines, 50 characters each. 13. The search feild must must include last name, city, state, birth, month and area code. 14. If multiple matching records are retrieved for a speicific search, display the summary of results. 15. User must be able to add, delete, modify and sort data. 16. When a new record is added it must be sorted alphabetically by a last name 17. The user will have to enter password to access the address book 18. For each user there will be a seprate application and address book ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request. 3) Exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform It should be written in Javascript.
