Adding Polls to website (php and mysql - and a bit of javasc

On my educational site I'd like teachers to be able to quickly and easily create polls that the students can do by means of selecting an option via a radio button, with the results displaying on submit. I've created the form where the poll can be created ( [login to view URL]) which is processed on [login to view URL], with data from this stored in the tbl_poll_titles and tbl_polls tables (sql dumps attached) TASK 1 - involves making some edits to these pages.
