Basic Form Web site with membership and comments, w/ MySQL & PHP

We need a simple web site on top of MySQL in PHP with the following pages and tables. The project must deliver final html using the same design as [login to view URL] and set up the full working site and databases on our hosted server delivering a final working site. The logic as designed should work, but we are open to sensible suggestions for improvements. Speed to complete is very important to us so please indicate ability to work on it immediately. EPISODE20 WEB SITE Summary • Flat welcome and message pages • Suggestions Form page open to public, for suggestions to be submitted • Advisors log-in and profile page • List of titles of all suggestions submitted • Suggestion comments page for advisors to comment on Suggestions submitted • Propose a friend page for advisors to recommend other advisors • DB Tables for all the above • Admin pages for administrator to edit, download or delete records in all tables ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
