Changes for site

I need some small urgent changes to an existing site. I have a site that basically exists of one page ([login to view URL]). see image balance_now.jpg. Now I want a different start page. All I want to do is this: See [login to view URL] 1. I have an flv file, that is a movie. I need a player (if this costs money I will buy it and send it to you.) that is top class and just starts as soon as the page is loaded. 2. I need a form with four fields that people enter and the data shall send an email as well as scave data in a table 3. I need a second form that does the same thing. 4. On an existing page where I enter info about "Please contact me" I want to add a drop down "How did you hear about us". Theis extra field shall be added to the existing table so that this new data is retrieved. (See [login to view URL]) ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
