Write an Android application

A council would like to develop an application that is used by health professionals, such as district nurses and health visitors, so they can refer to housing officers for their services. This application would be very simple in nature in that it will be a short form which can be sent to the houses officers internal database. The council would like the application to be able to identify the location of the sender, so the health professional can send the form while on a house visit and only needs to write in the name, telephone number, an optional email and select a few options such as money advice or fuel debt. This could be manually filled in or by voice recognition. The application will also have the details of the health profesional, which will be stored in the app when they log in. This will be automatically sent to the internal database.
