Android Application - Form entry that opens email and populates data when submitted from app

I need an application built that has multiple text box entry fields, certain fields must be filled to submit. Upon submitting from app the data entered on the form will be used to format the subject and body of the email and will always be sent to the same prepopulated email address. One Field I would like to have the end user be able to search for a client and retrieve the account pin and account manager contact information. This account lookup part I require the ability to update via an excel file that will always be formatted the same way. Don't need any login functionality. Would like simple splash screen when app loads initially. A simple company logo with brief loading bar. When submitted from app would like the end users to be able to select which email app installed on their device they would like to use.
