Shipping and Receiving Quality App

I am looking for an app that will allow my shipping and receiving departments to conduct a Quality Control process on a tablet. It will require a database to store up to 11 different fields including BOL# (or order #), Received From (or Customer), Date and time stamp, number of pallets received (or shipped), and a checklist of trailer safety checks including checking for bad odor, broken trailer, dirty trailer, Pests Present, contamination present. It will also require a comment field and a signature field. In addition, it should have the ability to take up to 10 pictures directly from the tablet (platform is Samsung Galaxy Tab) and store them in the record for future reference. I want a separate page to look up product on a webpage based on certain search parameters such as customer or order #. This should pull up the record for the transaction including the pictures. The resulting record (including pictures) should be able to be printed or emailed (PDF preferred).
