Desenvolver um Software aplicação/formulário -- 2

The project is a form consisting of several items that include: text, time (eg.: time from cardiac arrest to cardiopulmonar ressuscitation, etc), checkboxes; multiple choices and items with several sub-items and sub-sub-items. I would like that the sub-items options were comboboxes, eg.: - Initial evaluation (item) »» Esophageal temperature (sub-item): the options for this sub-item would be a number between 31ºC and 40ºC. »» Coma scale (sub-item): the options for this sub-item would be a number between 3 and 15. »» Ventilation (sub-item): the options for this sub-item would be: without any effort, irregular, etc. »» Pupils (subitem): size (sub-sub-item), symmetry (sub-sub-item)... the options for these sub-sub-items are 1 to 8 and yes or no, respectively. The form also contains a table which consists: - In the first row: the column headers (eg.: pupils, coma scale, electrocardiogram, etc) - In the first column: the row headers (the time the data were collected eg. 1h, 2h, 3h, etc) - In the remaning cells: the corresponding values (that vary with respect to the data they correspond, and that could be number or text). The form is small, it only has 4 pages. I would like the aplication to be able to run on android, windows pc and be able to export the data to excel. O projecto consiste em fazer um formulário que permita exportar dados para excell. O formulário contém entradas, como tempos (ex.: tempo de paragem cardiorespiratória até início de Suporte básico de vida), checkboxes, multiple choices e itens com vários subitens que têm opções e que eu gostaria que fossem combobox, ex: - Avaliação inicial (item) »» Temperatura esfágica (subitem) - Opções para este subitem variam por ex dos 31º-40º »» Escala de Coma (subitem) - opções para este subitem varia de 3-15 »» Ventilação (subitem) - opções para este subitem variam por ex: sem esforço nenhum,etc Para além deste tipo de entrada de dados, tem ainda um tipo de tabela que consiste: - Na primeira linha está o tipo de dados pretendidos (ex: pupilas, escala de coma, etc) - Na primeira coluna está as diferentes horas em que os dados foram colhidos (ex.0,1,2,3h, etc) - Nas células correspondentes estão os valores (que são muito variáveis) de acordo com o dado a que dizem respeito. O formulário é pequeno, tem apenas 4 páginas.
