I need a very simple Mobile app (Android) that will capture image from Mobile Camera and fill additional 2-3 fields (that comes from list of values LOV & free text) and Submit. These image & data should upload to server MySQL along with longitude & Latitude. User will get activation SMS while downloading this mobile app from Android Market.
