I need a C# / HTML / Javascript Application that can perform the work described in the enclosed document. We will want to build our own form interface that will use one standard form in CoreMotives and convey the information from multiple websites. Form shall be configured to allow for in script handling of forwarding the website visitor to a hidden field in the form.
