Form Design Project

I would like to design "Zip Submit" and "Zip OR State submit" forms to make them more attractive. I have attached the current forms here as a reference. As you can see, those are not attractive enough, and so the conversion rate is very low. I need some designer + coder to create better looking forms and then get them work in Wordpress. I will provide the code, you will need to set the approved design forms in wordpress in both JPG and PSD formats. The new forms size (width, height) should be similar to attached forms. Below are the requirements: There are two parts of this project: 1) Design and 2) Get it work 1) Design: Form 1 - "zip submit" form ( Form 2 - “Zip submit” + “State Select” See attached examples of both current forms. I need some design help to make them more effective to increase the conversion rate. 2) Get it work: You will need to make sure both approved forms actually work on wordpress, i.e. upon clicking 'submit zip' or 'submit state', the result page should show up. I will provide the form codes, all you need to do is embed the code in approved new forms and get them work.
