Create Needs Analysis Form / Contract fillable PDF

I would like to create a new needs analysis form for the finance industry. It should be neat and concise. It should flow smoothly, and progressively through relevant questions. Have room for all details and preferably be able to type inside boxes as well as tick boxes. I should be able to fill in details both electronically and manualy.
